医療の国際化に対応した人材育成を目指す大学教育のあり方に関す研究

研究概要

研究の背景

日本に中・長期滞在する外国人が増加し、保健・医療機関を利用する機会が増えている。また、国や神戸市は、いわゆる医療ツーリズムを行い、積極的に外国人患者を受け入れていく方針を出している。こうした事態に対応するために、保健・医療現場では、医療通訳やコーディネータの必要性が高まっている。そして、その育成のためには、民間教育機関だけでなく大学でも積極的な教育の必要があるとされている。ただその際、医療系学生には実用的な語学力や異なる文化への接し方を、また外国語学系学生には医療一般に関する知識や病気に苦しむ人たちの不安や苦痛への接し方を教える必要がある。しかし、これまで医療系学部や外国語部単独ではこうした要請に応える事が難しかった。そこで、今回、看護大学と外国語大学が協力して医療通訳・コーディネータの教育プログラムを開発し、それを共同授業等によって実践し、さらにそれを評価することによって効果を確認する実践的研究を行いたい。

研究実績・成果

本助成研究の後、平成24-26年度日本学術振興会科学研究費補助金基盤研究(B)一般に、（金川克子（研究代表者）「医療通訳・コーディネータの看護大学と外国語大学の共同による実践的教育プログラムの開発」）として採択され、研究を継続中であるので、あわせて報告する。

(1)国内外の医療通訳の現状及び教育プログラムの調査・分析

米国においても、医療通訳者の教育基準は未確立であるが、すでに多くの大学や大学院で医療通訳・コーディネータの育成が行われ、教育方法が蓄積されている。 例えばニューヨーク大学には医療通訳の専門コースがあり、デンバー大学には大学院修士課程に医療通訳の専門課程が設けられている。看護学の分野においても、異なる文化的背景をもつ患者への対処モデルなど様々な研究が行われている。またオーストラリア（クィーンランド大学）、英国（インペリアル・カレッジ）、カナダ（オタワ大学）、フランス（パリ大学）、スペイン（ジャウム大学）などでも医療通訳の育成が行われている。現地調査を行って、これらの大学や民間教育機関による医療通訳教育に参加したり、関係者にインタヴューを行ったり、研究会の講師として招いたりすることによって、教育プログラムを分析した。
またあわせて、先行する国内の医療通訳者育成プログラムを調査、分析した。りんくう国際医療通訳翻訳協会、インタースクールなど民間の教育コースのほか、大阪大学や東京外国語大学、愛知県立大学などの試みがある。こうした講座を実際に受講したり、講師を招いたりすることによって、教育プログラムを分析した。
　以上の結果、これらの教育プログラムは、文化的背景や制度の違い、看護学生が対象に含まれていないことなどから、そのまま利用できるわけではないものの、一定の基準を見いだすことが出来た。すなわち、最低30～40時間以上を必要とし、医学用語、疾病、通訳技術、倫理規定、異文化理解、ロールプレイ、実習、研修修了試験などが含まれていることが必要であることが判明した。また各授業の内容についても検討することが出来た。

(1) 神戸市の保健・医療機関を対象にした医療通訳の状況とニーズの調査

周知のように日本には国による医療通訳の制度はなく、医療通訳を雇用している一部の病院やNPO、国際交流団体と連携して医療通訳のボランティアを派遣している自治体（京都府、愛知県、神奈川県）などがある。そこで、各事業の担当者を講師として招いたり、聞き取り調査を行ったりすることによって、各事業の特徴や問題点について調査した。
それに基づいて神戸市の保健・医療機関に面接調査を呼びかけ、応じていただいた5施設を対象に聞き取り調査を行った。その結果、市民病院群ではNPO団体であるFACILとの提携で派遣型の医療通訳を実施しており、評価を受けていることが分かった。ただ、今後の問題として、コーディネータ料の負担や、救急対応等の問題があることが分かった。

(1) 全国の看護系・外国語系大学を対象にした医療通訳・コーディネータ教育の現状と展望

全国の看護系大学及び外国語系大学すべてを対象にして、医療通訳・コーディネータ教育の現状と将来について質問紙調査をした。その結果、現在、全国の看護系大学及び外国語系大学のなかで、医療通訳・コーディネータ教育を行っている大学は3校にすぎないこと、ただ将来的に検討したい、もしくは検討しつつあるという大学は約10校あることがわかった。自由記載の意見の中には、医療通訳の育成は専門学校に任せればよいという意見がある一方で、大学独自で育成をすべきだという声も複数あった。

(1) 教育プログラムの作成と実施、評価

以上の調査結果に基づいて、実践的な教育プログラムを開発し、両大学の教員を中心とするオムニバス形式の授業を行いつつある。2013年9月には、大学の学部制及び大学院生を対象にしたUnity単位互換講座（神戸市外大担当）として、「医療通訳・コーディネータ入門」を開講した。現在、約10人の学生が登録し、学習中である。また2014年2月1日（土）から4回にわたって市民向けのUnity公開講座（神戸市看護大担当）として、「医療通訳を知っていますか？」を開講する予定である。

今後の展望

現在開講中の二つの講座について、年度末に評価を行い、それに基づいて2014年度にも二つの講座を開講し、総合評価を行う。その結果に基づいて、報告書および「大学における医療通訳・コーディネータ育成の可能性」と題するシンポジウムを開催し、愛知大学、群馬大学、大阪大学など国内ですでに開講している大学からのシンポジストと、ニューヨーク大学など海外で医療通訳育成事業に携わっているシンポジストとでディスカッションを行い、今後の方向性について検討する予定である。
