更年期女性に対する健康教育プログラム作成の試み-植物性エストロゲンによるホットフラッシュ緩和の試み-

研究概要

本研究は、更年期女性を対象に、食事に関する健康教育プログラム（以後食事プログラムとする）を作成、試行し、食事プログラムの効果判定のための評価指標を探索することを目的とした。研究対象者は、ほてり体験を持ち、両側卵巣・子宮・乳房摘出術を受けていない女性とした。研究協力者は、女性センターに登録されている自助グループや婦人会を通じて該当者を募集し、参加協力の意思のある人とした。調査は2001年９月26日から10月26日に実施した。本介入プログラムは、植物性エストロゲンと植物性エストロゲン様の作用を持つ64食品を選択し、通常の食事に１日１食品を摂取することであった。調査期間は、１週間のほてり実態の記録に続いて、７週間の食事プログラムを実施する計８週間とした。食事プログラムの効果判定は、ほてり回数とほてりが生じる時間帯やその身体部位をもって主観的な症状の指標とした。主観的なほてり症状は、ほてり記録用紙への毎日の記録とプログラム開始前・４週目・７週目のほてり身体図によって把握した。また、血圧・心拍変動・心拍数の自律神経系の値をもって客観的な指標とした。さらに、ほてりの変化と食事プログラムに関する５回のグループ討議を実施した。グループ討議内容は、主観的な身体症状、食事プログラム実施を含めた健康行動と食事プログラムへの評価であり、この視点で討議内容を分析した。対象者の倫理的配慮として、研究主旨の説明は口頭と文書で行い、研究協力の自由意思を尊重し、語りたいことのみを語る権利の保証、各自がニックネームを用いたプライバシーの保護に努めた。調査は兵庫県立看護大学の研究倫理委員会の審査を受けて実施した。

調査協力を得た女性は６名で、平均年齢52.2±3.1歳、全員が主婦であった。調査開始時は、規則的に月経のある者１名、不規則な月経が２名、１年以上無月経の者は３名であった。主観的な症状としてのほてり回数は、食事プログラム開始前は１～41回／週であったが、食事プログラム最終週は０～８回／週と減少した。ほてりが生じる日常生活状況には、活動、食事、テレビ鑑賞時で、時間帯は午後が多かった。ほてり身体図による推移は、ほてり部位が限局あるいは縮小し、食事プログラム最終週にほてりが無くなった女性が多かった。グループ討議で語られたほてりの身体症状には、①発汗、②体の温かさ、③温度差への適応困難が述べられた。食事プログラムに対する評価では、楽に行えたという評価の一方で、食品を増やすことで体調を悪くしたという意見もあった。客観的な指標は、全プログラムを通して変化は認めなかった。心電図測定時にほてり症状が発生した女性がいなかったため、心拍変動のゆらぎが自律神経由来か否かは明らかにできなかった。しかし、女性のほてりが生じる生活状況には、交感神経優位の状況と副交感神経優位の状況が考えられた。そこで、ほてりが交感神経優位で生じるのか、副交感神経優位であるのかを心拍変動などの自律神経指標を用いて明らかにし、それをもとにほてりに対する看護ケアを検討することが必要であろう。

食事プログラムの効果判定のために、ほてりの身体図やほてり記録用紙を用いたことは、ほてりの主観的な症状の推移が把握でき、効果的な方法であった。今後の検討課題として、ほてりが生じる日常生活状況の「活動」内容の細分化を検討すること、食事プログラム実施期間の短縮と食事プログラム食品の摂取量に個別性を配慮することがあげられよう。さらに、自律神経系指標については、連続した心拍変動とほてり症状、季節変動とほてり症状との関連性などの検討が課題としてあげられる。また、更年期女性に対する健康教育プログラム内容には、ピアサポートの場を活用した日常生活行動変容も視野に入れたプログラムの必要性が示唆された。
